| Notes - (Value) | Referral Route: The referral criteria will be refined for this project but is based on the existing Housing First criteria used in Brighton and Hove. In each local authority area, partners will identify people who met the criteria using; ECINS, OPAL, Pathways, Mental Health and Substance Misuse Services, MARAC, Safeguarding Boards and High Impact Casework Forums. The lead officer will work with Fulfilling Lives Service User Groups and other services in each area to develop a robust criteria and referral form for this programme to be used all the agencies making referrals, ensuring a comprehensive list. The Panels will build on pre-existing multi-agency relationships and working arrangements. |
